Cagrilintide (Cagrilintide (AM833))

Category: Weight Management. Status: Investigational (Phase III).

A long-acting amylin analog being investigated for obesity, both on its own and in a fixed-ratio combination with semaglutide (CagriSema). Amylin is a hormone co-secreted with insulin that promotes satiety and slows gastric emptying.

How it works

Acts as a dual amylin and calcitonin receptor agonist. It engages satiety centers in the area postrema and hindbrain, reducing food intake and slowing gastric emptying through a pathway distinct from GLP-1.

Key facts

  • Molecular weight: ~3963 Da
  • Half-life: ~7-8 days
  • Bioavailability: High via subcutaneous injection
  • Storage: Lyophilized: refrigerate. Reconstituted: 2-8°C, use within ~30 days.

Dosing overview

  • Typical dose: 0.3-2.4 mg (titrated)
  • Frequency: Once weekly
  • Duration: Ongoing while needed
  • Route: Subcutaneous injection

Protocol notes

  • Trials titrate slowly from a low dose (e.g. ~0.3 mg) upward over several weeks to limit nausea.
  • Once-weekly dosing is enabled by the long half-life; a fixed weekday is typical.
  • Often studied alongside semaglutide (CagriSema) to combine amylin and GLP-1 satiety pathways.

Reported benefits

  • Appetite suppression and increased satiety
  • Meaningful weight loss
  • Complements GLP-1 agonists (additive effect)
  • Slows gastric emptying

Possible side effects

  • Nausea (dose-dependent)
  • Reduced appetite
  • Injection site reactions
  • Constipation or GI upset

Research

  • Cagrilintide once weekly for weight management (2021): Phase 2 trial showed dose-dependent weight loss up to ~10.8% at 26 weeks vs ~3% placebo.
  • CagriSema (cagrilintide + semaglutide) in obesity (2023): Combination achieved roughly 15-17% weight loss, exceeding either agent alone.

  • josh_mobility rated it 3/5 - Cagrilintide took the edge off, not much more: I ran cagrilintide for about 4 weeks while I was trying to trim down a little without wrecking my lifting, sitting at a desk all day in South heat and being snacky as heck, and it did help me feel full faster. I dropped about 4 lbs, which is solid but not some huge shift. For me it felt like a quiet assist, not a dramatic change, and I’d still want to see better data before I get excited about it.
  • jason482 rated it 2/5 - Cagrilintide blunted appetite but left me queasy: I ran Cagrilintide for about 12 weeks while trying to trim down after years of shift work and wrecked routine, living in Ireland now and training around it. I lost about 2 kg, but the queasiness and heavy stomach feeling were grandly annoying for me. That was the main letdown.
  • robin_n rated it 3/5 - Cagrilintide felt subtle, not dramatic for me: I tried cagrilintide for 4 weeks while I was juggling startup stress and trying to keep my eating less chaotic in a Midwest winter. By week 1 I noticed I was getting full faster. Weeks 2 and 3 felt steadier, and by the end I was down about 3 lbs.
  • megan302 rated it 4/5 - quieter appetite than semaglutide, but a little too sticky: i ran cagrilintide for about 3 weeks while i was trying to get my eating back under control after a sloppy stretch of travel and too many late dinners here on the west coast. for me, it did blunt hunger pretty clearly, and i lost about 3 lbs. i actually preferred semaglutide for this goal, though, because cagrilintide felt a little more like my stomach was just sitting there. still, i’d give it 4 stars for appetite control.
  • josh_coldplunge rated it 3/5 - Ok appetite control, but semaglutide worked better for me: I tried cagrilintide for about eight weeks, mainly to deal with late-night snacking during long nurse shifts here in São Paulo. I lost around 3 kg, but honestly, semaglutide seemed stronger at keeping my hunger down. For me, cagrilintide was decent, just less effective overall.
  • weekendsanjay rated it 4/5 - Quiet appetite, fewer second breakfasts: I tried Cagrilintide after my post-shift snacking got a bit stupid, eh. Between physio days and the gym, I was always grazing. For me it made meals feel finished, and over about 6 months I dropped 17 kg. Mild nausea showed up early, then settled.
