Metformin & Aging: TAME Trial Results Make Me Hopeful but Cautious

I just finished reading the latest TAME interim data that came out in the Journal of Gerontology. The paper shows that 500‑1000 mg of metformin slowed biomarkers of aging in people 65‑85 in a 2‑year randomized controlled trial. The effect on cardio‑metabolic endpoints was modest, but the shift in inflammatory markers and mitochondrial function was more impressive than I expected. It’s tempting to treat metformin like a magic bullet, but the authors remind us that it’s still early and that lifestyle factors were the real drivers of change.

In my own practice I’ve been adding 500 mg twice a day for the past year to my clients with type 2 diabetes, and I’ve seen a noticeable drop in fasting glucose and a slight improvement in Brookier's test scores. I’m not sure if those cognitive gains are real or just a placebo effect, but it’s interesting.
