Trying Telmisartan for Blood Pressure and Energy – First Week Experience

I just started a 20 mg daily dose of telmisartan to keep my blood pressure in range and was curious about any extra perks it might give me for energy and metabolic health. Telmisartan is an ARB, but it also has a partial PPAR‑gamma agonist effect, so I’ve read that it can improve insulin sensitivity and equips mitochondria a bit better. After the first week my cuff shows a reduction from 140/90 to about 130/85, which is nice dared.
I also felt a modest lift in my usual fatigue after a long shift – not a miracle, just a slight change in how I wake up, perhaps from better blood flow or the PPAR‑gamma action. No dizziness or low blood pressure yet, so that’s a relief. I’m not taking metformin or rapamycin at the moment, but I’ve had a conversation with a friend who uses both.
Has anyone else tried telmisartan for longevity purposes, or mixed it with a low‑dose rapamycin or metformin stack? Any observed interactions or advice on timing would be great.

- rational_squats: For what it’s worth, I started telmisartan a couple of years ago for high blood pressure. The drop in the cuff was similar to what you’re seeing, about 10‑15 mmHg in systolic. I also noticed a slight lift in morning energy, probably just better perfusion after a long night shift. I’ve never mixed it with rapamycin or metformin, so I can’t say how they interact, but I’d keep a close eye on blood pressure and liver enzymes if you do. Do you have any experience with how it affects appetite or slee
- hank_m: Tbh i've noticed a very subtle dip in appetite – just a bit less snacking after lunch, probably because my glucose is steadier – but nothing dramatic, no crash, and sleep hasn't changed much, just a little lighter first night and a few early wake‑ups. Still keep an eye on BP and liver if you mash it with rapamycin or metformin, yeah.
- hank_m: Tbh i haven’t noticed any appetite shift with telmisartan, nothing different from my usual eating pattern, and the sleep’s been pretty normal – just a little easier to wind down after the dose, but MBA it’s been too early to call it a pattern. Still sticking to the blood‑pressure checks and liver‑enzyme checks Pat, just in case.
- greg208: Glad to hear telmisartan’s not messing with your appetite and that the wind‑down is a bit easier. I’m keeping a log of my BP and doing a quick LFT panel a month in, so I’ll be watching the liver marks closely. Got any tips on the ideal frequency or dosage of LFTs when you’re on an ARB? Also, have you seen any subtle changes in energy after a week?
- greg208: Thanks for the heads‑up, hank_m. I haven’t noticed any appetite swing so far – my snacking hasn’t dipped since I started telmisartan, so maybe you’re more sensitive to the glucose stabilisation. I’ll keep a log of my BP and liver markers if I ever add rapamycin or metformin. How often do you check liver enzymes when you’re on the combo?
- greg208: You’re spot on about the 10‑15 mmHg drop – mine was 10 mmHg systolic, so we’re matching. I haven’t noticed any appetite swing yet, my meals stay the same. Sleep hasn’t been altered either – I still tô run 8‑h cycles. I’m yet to order liver enzymes, but thinking of a quick pre‑ and post‑treatment check after three months. How do you log BP on night shifts – a cuff every few hours or a continuous monitor? Any tips on timing the dose for best nocturnal perfusion?
