RU-58841 scalp irritation – how to minimise it?

Posted by greg208 in Safety & Side Effects - 1 points, 10 comments.

I started RU‑58841 a few months ago to try and stop the hair thinning that’s been creeping up. I followed the usual 50‑100 mg dose in a 1 ml vehicle, applied it twice a day to the scalp. The first week I felt a sharp itch and a little burning sensation that made me question if I’d gotten the wrong product.

By the third week the itching had become a constant tickle, the scalp felt dry and a few tiny red bumps popped up – clear signs of contact dermatitis. I stopped using it for a week and noticed the irritation ease a lot. When I resumed, I tried mixing the cream with a mild, fragrance‑free moisturizer and also added a small amount of 4 % hydrocortisone cream on the most irritated spots during the night.

That seemed to cut the itching in half. I also made a small patch test each new batch of product before full application – a few drops on the inner wrist, wait 24 h, and if there’s no reaction, it’s probably safe for the scalp. I’ve not seen any serious side effects yet and the hair growth looks slightly better, but I’m still learning about the long‑term safety.

Anyone else have experience with scalp irritation from RU‑58841? What skincare routine helped you?

  • hank_m: Tbh i’m in a similar spot, i tried RU‑58841 a while back and got a nasty itch on the first try, then the bumps, and the good news is i found a way to cut it down a bit. I mixed it into a fragrance‑free aloe gel instead of a straight cream, kept the dose lower at 40 mg a day, and really made a habit of applying a thin layer of 2 % hydrocortisone ointment only where the trouble spots were, then wiping it off the next morning – that helped the itch feel less constant. I also keep a small vial of a
  • optimizer920: I’ve been on RU‑58841 for a while now and the itch was real at first. I started at 75 mg in a 1 ml gel and mixed it 1:1 with a fragrance‑free silicone base. 5 % niacinamide to the base – it softened the surface and kept the scalp hydrated. I still do a wrist test before each new batch. The only thing that really cut the redness was using a thin layer of 1 % hydrocortisone at night, but I stop it after a week to avoid masking the irritation. No serious side effects yet, but I keep track of how m
  • hank_m: Tbh that hydrocortisone trick is pretty spot on, i’d seen a similar tickle fade after a short burst of 0.5 % instead of 1 % though i think the smaller dose still keeps the irritation real but silences the redness, so i’ll try that on my next batch. I’ll keep the wrist test though, it’s the easiest sanity check.
  • coastal_medic: Sounds spot on – the aloe base really softens the sting. I’ve seen hydrocortisone help a bit too, but I keep the dose 20 mg a day and never leave the ointment on overnight. Have you noticed any change in hair thickness since you tweaked the routine?
  • hank_m: Thanks optimizer920, i can relate to the early itch and the red bumps, i also mixed it with a fragrance‑free silicone base but started at 50 mg, 1:1, and the 5 % niacinamide helped mellow the surface a bit, though i finally added a thin layer of 0.5 % hydrocortisone at night for a short test. I keep a daily log of scalp feel, and i pause the steroid after a week just like you, because i’ve found the itch only eases when the scalp isn’t being masked. Keep up the tracking!
  • greg208: Thanks mate, glad we’re on the same page about the itch. I’ve been using a fragrance‑free silicone base too, but I didn’t mix in niacinamide – that sounds like a solid tweak. 5 % hydrocortisone night works for me too, but I only keep it for a week; after that the scalp still feels a bit raw. Do you notice the bumps clearing faster once you stop the steroid executor?
  • greg208: I’ll keep the 20 mg limit you mentioned and only spot‑treat at night, not all night. I haven’t seen any noticeable change in density yet – it’s been about a month since the tweak and the hairs still feel thin. How long did you wait before you first noticed a thicker look? Any other tweaks you’d suggest while I’m still in the early weeks?
  • greg208: Thanks for the tweak, hank_m. 5% on my next batch and see if the itch drops further while still keeping the redness under control. I’ll stick to the wrist test on the inner forearm for consistency. Have you noticed any difference in how long it takes for the skin to calm down with the lower dose?
  • greg208: Great to see a similar approach – I used 4 % hydrocortisone on the worst spots, so 1 % should keep it gentler. I’m curious about the silicone base you use – is it a specific brand or just a neutral silicone gel? Also how do you quantify the daily scalp feel? I’ve been jotting notes on itching and dryness, but any extra metrics you track would be handy.
  • greg208: Nice that the aloe gel trick worked for you. The 40 mg a day down‑dosing and the spot‑application of 2 % hydrocortisone, wiped off in the morning, sounds like a sensible compromise impose. I’ll try that, but I wonder if the lower dose affects the hair‑growth benefit, any data on 40 mg efficacy so far?
