Titrating Tirzepatide: How I Managed Nausea and Found My Sweet Spot

5mg once a week because I’d read that low and slow helps with the GI side effects tbh. First week I felt a bit queasy after dinner but nothing that kept me from eating, just a dull nausea that faded by morning. I kept my usual meals and tracked my appetite with a simple notes app, noticing I was full faster especially after protein.

At week four I bumped to 5mg and the nausea came back stronger for about two days then settled, my hunger dropped noticeably and I started losing about a pound a week without changing my gym routine. 5mg and the nausea lingered longer, plus I noticed a bit of fatigue in the evenings, so I stepped back to 5mg where I felt steady. My HRV stayed stable throughout and my fasting glucose dropped a few points, nothing dramatic but a trend I liked.

Has anyone else found that pausing at a mid dose works better than pushing higher? I’m thinking of holding at 5mg for a while before considering another increase.

  • grace_sleepnerd: I’ve been on tirzepatide for a couple of months now, so I can say the pacing feels important. 5 mg and doubled every 4 weeks, but when I hit 5 mg the nausea spiked for two days before it settled. 5 mg. My weight loss slowed a bit, but my appetite stayed more controlled and my HRV stayed steady. I’d ask, did you notice any change in your sleep or energy when the nausea cleared?
  • amber464: I didn’t see a big swing in my sleep quality – the nights stayed about the same. Energy dipped a little on the first couple of days after the nausea hit, then rose back once theawrich went away. My HRV nudged up a bit, but I kept it steady.
