Injection site swelling with Thymosin Beta‑4 – any tricks to minimise it?

I started using Thymosin Beta‑4 (TB‑4) at 3 mg twice a week about six months ago for a bruised knee from a fall. The first few injections were fine, but after the third round I noticed a nice thick swelling around the site that lasted a day or two and made the area feel tender when walking. I also felt a bit of mild fatigue that week and a headache the next morning that never bothered me before. I’ve been reading the side‑effect list and it says injection‑site swelling, temporary fatigue, and mild headache – which is what I’m seeing. I switched to rotating the injection spot each time and tried a slightly lower dose of 2 mg, but the swelling still shows up.
I’ve got a routine of cleaning with alcohol, warming the skin, and applying a light pressure bandage for 15 min after injection. It seems to help a bit, but not eliminate the puffiness. Anyone else have used TB‑4 and found a way to keep the swelling down? Or have you seen the fatigue and headache go away with a different schedule? Any tips would be much appreciated.

- liam_sleepnerd: I’ve been injecting TB‑4 at 3 mg twice a week for about a year for my own knee pain, and the swelling was almost the same as yours. What helped me was changing the needle length to 30 mm instead of 25 mm; a longer needle gets the peptide deeper, so it doesn’t pool under the skin. I also dilute the vial with a little saline before injecting, which slows the flow and reduces the local reaction. I started spacing the doses to three days apart instead of consecutive days and the fatigue and headach
- sarah_c: Are you sure about the longer needle? I would think a shorter needle would be safer for subcutaneous injections so you do not hit anything you should not, eh? Fwiw, I have not used TB-4 myself, but I usually prefer a smaller volume of liquid to avoid that pooling feeling. Do you have a study or a guide that suggests 30 mm for this?
- greg208: I was using a 23 g 15 mm needle with a 2 ml syringe, aiming to stay subcutaneous. 30 mm felt too long for me; I thought it might go deeper, but the swelling still happened. I already dilute the steroid to 2 ml so the volume is low. I’ll try a 12 mm needle and see if the puffiness drops. Do you have a source that recommends 30 mm for TB‑4?
- greg208: Thanks for the extra tips, liam_sleepnerd. I’ll swap to cấu 30 mm needles, dilute the vial with a bit of saline, and space the doses to three days apart like you do. The 20‑minute cool compress sounds solid – how long after the injection do you wait before you start the compress? Also, what volume of saline do you add?
