Pentosan Polysulfate oral + rehab for stubborn knee OA – dosing and timing query

I’ve been battling medial knee osteoarthritis for a few years now, and the pain spikes whenever I try to increase my hill repeats. A few months ago I started a low‑dose oral Pentosan Polysulfate regimen – 100 mg three times a day – after reading about its cartilage‑support properties. I’m also doing a structured physio programme with isometric quad work and light cycling on off‑days.
What I’m trying to work out is the best timing around my rehab sessions. I’ve taken the first dose with breakfast, the second right before my physio (usually mid‑morning), and the last with dinner. I’ve noticed a slight reduction in joint ache after about two weeks, but I’m not sure if the timing is optimal or if I should split the doses differently on training days.
Has anyone combined oral Pentosan with regular knee rehab and found a particular schedule that seems to help? Any tips on managing the occasional mild stomach upset would also be appreciated. 🌱

- mateo_n: anecdotally i’ve taken pentosan with a full glass of milk or a small snack mid‑day and it helped keep the stomach from throwing a fit, so i’d suggest doing that on training days. my own routine was breakfast, lunch, and dinner, same split you’re using, because the drug’s bioavailability is pretty steady when you keep a consistent routine. i didn’t see a big difference when i moved the midday dose to right after my hill repeats; the joint pain stayed about the same, so i’d lean toward sticking wi
- aspiring_trailrun: Thanks mate, I’ve had the same stomach bug with the breakfast and dinner doses so I’ve moved the mid‑day 100 mg to a small snack with a glass of milk, exactly as you did. I haven’t yet tried taking it right after a hill repeat – I’ll give that a go next week and see if it tweaks the pain level. Any thoughts on splitting the dose into 50 mg halves? 😎
