Tirzepatide titration from 5 to 7.5mg, resting HR and HRV changes

Been on compounded tirzepatide for about 11 weeks now. Started at 2.5, went to 5 for four weeks, just bumped to 7.5 last friday. Tracking the usual stuff with my whoop and a glucose monitor.

One thing i noticed going from 5 to 7.5 was my resting heart rate dropped about 4 bpm on average. Was sitting around 58-60, now hovering 54-56. HRV went up too, maybe 5-6 ms on the weekly average. Could just be less overall stress from carrying less weight? Hard to say. Appetite suppression definitely kicked harder at 7.5 though, ngl. First couple days i had to remind myself to eat which felt weird.

Sleep's been sweet as actually, deep sleep percentage ticked up a bit. No nausea at 5mg but day two after the 7.5 bump i felt a bit off, nothing major. Glucose readings have been flatter, fewer post-meal spikes, which lines up with what you'd expect from the GLP-1/GIP dual action.

Keen to hear from anyone who's gone up to 10 or 12.5, did the HR trend continue or did it plateau out? Also wondering if anyone's tracked their lipids partway through, i've got bloods due in about three weeks and curious what to expect.

  • curious_scientist: I noticed something similar when I went from 5 to 7.5. My resting HR dropped maybe 3 bpm, hovered around 55 for a while. But at 10 it didn't really keep going down, just stayed there. Felt like it plateaued. HRV bumped up a little more but nothing dramatic, maybe 3 ms. Could've been weight loss too, hard to separate. On lipids, my triglycerides came down noticeably around the 3 month mark. LDL didn't move much for me. HDL ticked up slightly. Just my own panel though, your mileage could vary. Th
  • greg_codes: Yeah that plateau at 10 makes sense, kinda what i'd expect once the weight stabilises a bit. The HR drop probably has diminishing returns once you're not shedding as fast. Keen to get a panel done around the 3 month mark actually, haven't pulled bloods since starting. Triglycerides dropping would be sweet as, mine were a bit elevated beforehand. Not tracking fasting insulin yet but i should, got a metre that does it. You seeing the insulin come down alongside the glucose flattening?
