Compounded GLP-1 safety gaps are getting worse, not better

Partnership for Safe Medicines just put out a May 2026 update on GLP-1 safety risks. The main point is that some compounding pharmacies classified as small-scale 503A operations are now producing semaglutide and tirzepatide at manufacturer-level volume, and regulators cannot keep up with monitoring that scale.
This squares with what I have been seeing anecdotally. People I know who have used compounded semaglutide have had wildly different experiences with potency and side effects between batches and pharmacies. I track my own sleep and HRV pretty closely, and I reckon inconsistent dosing would show up in those metrics pretty fast if you were underdosed one week and hit hard the next. The article does not really dig into what that means for the end user though, which is the part I actually care about.
Does anyone here who has used compounded GLP-1s noticed differences between batches that made you question what you were actually getting? Keen to hear if anyone has paired their dosing with bloodwork or wearable data to catch inconsistencies.

- hank_v: I have not used compounded GLP-1s myself so i cannot speak to batch differences directly. But the volume scaling issue you describe is actually a real concern from a chemistry perspective. When you move from small-scale compounding to manufacturer-level output, the quality controls that matter most (homogeneity of the final product, sterility, potency verification per batch) become much harder to maintain without the infrastructure that proper manufacturers have.
