Oral BPC-157 for tendon rehab, is it actually doing anything systemically vs just gut?

I've been running injectable BPC-157 for a lingering rotator cuff issue, around 250 mcg twice a day subq near the shoulder. Anecdotally it seemed to help with the sharp pain on overhead movements after about two weeks, though I can't say for sure it was the peptide and not just time and rehab work.

Here's my question. I've seen people switch to oral BPC-157 arginate for convenience and claim it still helps tendons systemically, not just gut. The profile on this site says oral has lower potency than injectable and the main benefit is direct gut contact. So if my target is a shoulder tendon, am I just wasting oral BPC on GI effects that never reach the joint?

I get that some systemic absorption happens orally, but is it enough to matter for tendon repair? Anyone here actually switched from injectable to oral for a joint or tendon issue and felt the difference either way? Not looking for anyone to tell me what to take, just trying to figure out if oral is a real option for this or if I should stick with pins.

  • patient_mito: I looked into this same question a while back because I was tempted to switch from subq to oral for convenience. The honest answer is I could not find solid human pharmacokinetic data showing oral BPC-157 reaches tendon tissue in meaningful amounts. Most of the systemic claims seem to come from rat studies, and the dosing there does not translate cleanly to humans. For me, subq near the injury site seemed to do more than oral ever did, but that is just my own experience and could be placebo or
  • frugal_fasted: I had the same thought when I was trying to sort out a stubborn Achilles issue. I ended up sticking with the subq because the oral stuff just felt like a waste of money for anything outside the gut, imo. It felt much more direct near the actual injury. Did you find any specific dose for the oral version that actually felt useful?
  • fasted_scientist: Ngl, I’m with patient_mito on this – the human PK data just isn’t there for oral BPC-157 getting to shoulder tissue. I tried oral arginate for a few weeks (gut‑focused dosing) and felt nothing in my cuff, only the usual gut comfort. Has anyone actually tracked tendon thickness or pain scores with oral‑only? Would be good to see.
  • tracker_jade: That pretty much confirms what I suspected. You felt gut effects but nothing in the cuff, which lines up with the idea that oral just doesn't get enough systemic concentration to a shoulder tendon. Anecdotally my injectable seemed to do something for the pain but I never tracked anything objective like tendon thickness on imaging. Did you do any ultrasound before and after your oral run, or was it just symptom tracking?
  • tracker_jade: Here's a thought: I went oral for a few weeks after running out of pins, same 250 mcg dose split morning and night. Honestly, I didn't notice any difference in the shoulder at all. The joint still felt the same after three weeks, so I went back to subq. Maybe it's just me, but the oral route never seemed to register on my tendon pain. Did you try a higher oral amount, or just stick with the standard dose?
  • tracker_jade: Yeah the lack of human PK data is kinda what drove me to ask here in the first place. The rat studies get thrown around a lot but you're right, dosing doesn't translate cleanly and I don't trust extrapolation from those models for something like tissue distribution. Your experience lines up with what I'd expect honestly. Subq near the shoulder seemed to do something for me in that two-week window, and I'd be surprised if oral matched that for a specific tendon target. Did you stay with subq the
