Pentosan Polysulfate oral for knee tendon pain – timing around PT?

I’ve been dealing with nagging patellar tendon pain from standing all day in the classroom and light jogging on weekends. I’ve read that Pentosan Polysulfate might help with cartilage support and inflammation, and I’m considering trying the oral 100 mg three times a day regimen. Anecdotally, has anyone here used it for tendon issues rather than just bladder or osteoarthritis?

If so, did you notice any change in pain or swelling after a couple weeks, and how did you time the doses around your physical therapy sessions? I’m trying to figure out whether to take it with food or on an empty stomach to avoid GI upset, and whether splitting the doses throughout the day works better than all at once. Also, did anyone run basic labs (like CBC or liver panels) to check for bleeding risk before starting? Thanks.

  • fasted_scientist: Ngl I'd push back a bit on the premise here. Most of the PPS data for joints is from intra-articular or IM use in veterinary work (horses mainly), not oral dosing in humans. The oral bioavailability is pretty poor compared to injectable, so I'd be sceptical you'd get meaningful tendon remodelling from 100mg TID orally. That said, I haven't seen solid human RCTs for oral PPS in tendinopathy specifically, so if anyone has one I'd genuinely want to read it. On the labs question, a CBC won't tell y
  • retired_vo2max: Yeah the bioavailability thing is real, I looked into this a while back and came to the same conclusion. Most of the human oral PPS studies that actually showed something were osteoarthritis, not tendon stuff, and even those were kinda mixed. I tried oral PPS for a cranky patellar tendon a couple years back and honestly didn't notice much. Could've been underdosed for what I needed though, or just not enough time.   • tracker_jade: Thanks for the honesty on the patellar tendon part, that's kinda what I was worried about. Most of what I found was osteoarthritis too and I wasn't sure if anyone had actually tried it for tendon specifically. Good to know about the food thing though, I'll definitely take it with meals if I go for it. How long did you run it before you called it? And do you remember what dose you were at? Just trying to figure out if there's a minimum timeframe before you'd even expect to notice anything.

Anecdotally I just see a lot of people claiming it works orally but nobody posting details. Good catch on the labs too, you're right that PT/INR and aPTT are what actually matters there. Did you ever look into the injectable route yourself, or did you write off PPS entirely based on the lack of human data?
