My 6‑week CagriSema titration: appetite dip, nausea spikes and a weird HRV bump

I started CagriSema about a month ago after reading about the combo on the forum. I began at 0.3 mg total (0.15 mg each component) weekly and added 0.3 mg every two weeks until I hit the 2.4 mg/2.4 mg target at week 6. Weight dropped 4.8 lb in that time and my daily calorie intake fell from ~2100 to ~1500 without me really trying to count.

The appetite suppression was real for me – I barely felt hungry after lunch and skipped the usual afternoon snack. Nausea was the biggest side effect; it hit me hard on nights 2‑3 after each injection, lasting 6‑8 hours, but it faded after about week 3.

What’s odd is my HRV: baseline 48 ms, then jumped to 60 ms by week 4 and has stayed around there. I’m not sure if it’s the amylin component, better sleep, or just a placebo.

Anyone else see a similar HRV trend or have tips for handling the nausea?

  • leah_vo2max: to be fair the nausea usually hits when the glucose levels dip too fast, i had the same thing on a similar stack and drinking electrolytes like crazy helped a bit. the HRV bump is weird though, maybe it is just because you are sleeping better since the inflammation is down, but i wonder if the amylin part is affecting your vagal tone. did you notice any change in your resting heart rate too? i felt like my RHR dropped a bit when i was on a GLP-1 combo, which usually pushes HRV up. you should d
  • coastal_gains: ngl leah same here – nausea hit hard when my glucose dropped fast, sipping electrolytes took the edge off. My RHR did dip a few bpm and HRV jumped, felt like better sleep. Just tracking labs now to be safe. 🧠
  • amber464: Yeah, totally feel you on the nausea hitting when glucose drops, sipping electrolytes (I’ve been mixing a pinch of sea salt + a splash of lemon in water) has taken the edge off for me too. My RHR dropped a couple of bpm and my HRV jumped up a notch after the first week, which honestly made me feel like I was sleeping deeper even though I wasn’t tracking sleep yet. I’ve started pulling a basic panel every two weeks (CBC, CMP, fasting glucose, and a quick lipid panel) just to keep an eye on thing
  • amber464: my rhr actually stayed pretty flat, maybe dropped by 2 or 3 beats but nothing crazy. imo the hrv bump was probably just better sleep since i stopped snoring so much. the electrolytes definitely helped with the nausea though. i'm still tracking my fasting glucose every morning just to be safe and keep a close eye on the trends.
