TB-500 timing around PT for rotator cuff strain – any tips?

I’ve been dealing with a nagging rotator cuff strain for about six weeks. I’m seeing a PT twice a week and doing the usual band work and scapular stabilizers. 5 mg subQ twice a week, injecting the night before my PT sessions.

Anecdotally I felt a bit less stiffness in the morning and the pain during external rotation seemed to drop a notch after the first week, but I’m not sure if that’s just the PT kicking in or the peptide. I’m wondering if anyone has tried timing the dose differently – like injecting right after PT or on off days – and noticed a clearer effect on soreness or range of motion. Also curious if anyone split the dose into smaller daily amounts instead of twice weekly.

Would love to hear what’s worked for you guys for timing and any side effects you saw. Thanks.

For me, injecting the night before seemed to work well, but I actually found that splitting the dose into more frequent, smaller amounts felt better for keeping things steady. I tried doing it every other day instead of twice a week and it seemed to help with that morning stiffness, though it is just my own experience. I did notice a
  • tracker_jade: I’ve been thinking about trying every other day instead of twice weekly to see if the steadier level helps with morning stiffness.
  • cole_l: Tbh I tried splitting my dose into smaller shots every other day and noticed the morning stiffness eased a bit more steadily. The skin irritation I got was just from pinning the same spot too often – rotating sites helped. Just my own experience though.
  • tracker_jade: That's helpful, thanks. Did you stick with the same total weekly amount when you split it, or did you lower it a bit? I'm at 10 mg total per week right now and wondering if splitting that into smaller every-other-day shots would be too much or about right. Also good call on rotating sites, I've been using the same two spots mostly so I'll mix that up.
  • nof1_aisha: I kept the same total 10 mg when I moved to every‑other‑day 2.5 mg shots, didn’t lower it, and felt a bit steadier soreness drop. Rotating sites helped avoid lumpiness. You could try 2.5 mg EOD and see if the morning stiffness eases more than twice‑weekly 5 mg. Just my experience.
