TB-500 + BPC-157 timing around PT, what actually worked for my Achilles?

Tore my left Achilles last spring, partial tear, did the whole rehab thing. Started TB-500 and BPC about 4 weeks in once the acute phase calmed down.

The thing I'm curious about is timing. Loading was 2x/week and I kept PT on the same days, then moved to maintenance once I was walking normal. Seemed to make a difference when I pinned close to a session vs the day before. Hard to say if that was real or just me wanting it to be.

I'm like 9 months out now. Tendon still feels "thick" if that makes sense. Not painful, just not the same texture as the other side. Mobility is fine, single leg calf raise is back. But I notice it on stairs sometimes.

Has anyone run a longer maintenance phase for stubborn tendon remodeling? I keep reading about actin upregulation and cell migration with TB-500 and wondering if a slow low dose for another couple months would actually help the remodeling finish. Or am I past the window where it does much.

  • sofia_n: Genuinely curious about this one because my own Achilles rehab is like 14 months out and I still get that "thick" feeling you're describing, totally get what you mean by the texture thing. The actin stuff with thymosin beta-4 is mostly in vitro and rodent tendon models though, anecdotally I tried a low maintenance phase (about 2mg once a week) for 6 weeks and didn't notice a dramatic shift but it could just be placebo or that I was already pretty far out.   • zoe_rows: Appreciate the honesty on the 6 week run, that's actually useful since you're farther out than me. The 2mg once a week lines up with where I was thinking. To be fair I wasn't expecting a dramatic shift either, more like nudging the remodeling along. What was your timing around activity on those doses? Same day as training or before/after? Curious if that's where I might have been missing something. Also yeah the in vitro caveat is fair, I know most of this is animal model stuff. Just hard to f
  • tiredmira: Yeah the remodeling timeline is real, my PT kept saying tendons are slow and basically the last tissue to finish adapting. 9 months felt early honestly. The texture thing never fully went away for me either but it stopped bugging me around month 11. Could be scar just settling in. 🥗
  • zoe_rows: Month 11 is reassuring to hear, thanks. The scar settling thing tracks with what I've been feeling, just kinda lumpy compared to the right side. Did your PT give you any cue that the texture was actually changing over time, or was it more of a subjective "feels less weird" kind of thing? Mine just shrugs when I ask.
