GLP-1 patients reporting muscle loss, what does the imaging research say now

Anyone else following the back-and-forth on lean mass loss with GLP-1s lately? I keep seeing threads about people losing weight on these meds but their DEXA scans looking worse than expected, and it seems like the research community is finally catching up.

There was a review that came out recently questioning whether the standard "lose fat, keep muscle" assumption actually holds up when you look at body composition data instead of just the scale. If the muscle loss turns out to be clinically meaningful, that changes the whole conversation about dosing speed and protein intake.

Personally, my own DEXA before and after showed more lean mass drop than I was comfortable with. I had to seriously bump my protein and add resistance training to course-correct. Could be placebo, could be the rapid titration, could just be me, but the numbers didn't lie.

For those who track body comp closely, have you seen similar? And what does your training look like to protect lean mass while staying in a deficit on these meds?

  • quiet_sleepnerd: Yeah I've been watching this anecdotally and it lines up with what people around me have said. Anecdotally most folks I know on them dropped scale weight fast but a couple got DEXA scans and were a bit gutted by the lean tissue numbers. Slower titration seemed to help one mate, plus he stacked his protein earlier in the day and kept heavy lifts in. I reckon the dosing speed matters more than people think, and resistance work isn't optional on them, it's the whole defence really.
  • frugal_protocol: The titration point is actually what caught my attention since i have been reading the same threads. For what it is worth, the people i have spoken with who pushed through too fast seem to be the ones who ended up with the worst body comp results, regardless of how much protein they ate. Resistance training i completely agree with you, it really seems non-negotiable on these.

Did your mate's follow up scan show any recovery once he slowed the titration, or was it still a net loss overall?
  • tiredmira: Honest_nomad, no mate here, that was my own scan πŸ˜… But yeah, after I slowed the titration and locked in protein around 1.6g/kg plus heavier lifting, my follow up DEXA 3 months later showed partial recovery. Not back to baseline but trending up. Took longer than I expected tbh. Curious what your grim scans looked like compared to total weight lost, like was it proportional or worse than expected?

I titrated faster than I should have because I wanted the appetite suppression to kick in, and I think that backfired on the body comp side. Heavy lifts being non-negotiable, yeah that's basically the takeaway. Did your mate's DEXA numbers actually recover or was it more of a "stop the bleeding" situation?
