HCG and estradiol: how high is too high before you worry?

Ive been on HCG for a few months now, around 500 IU twice a week alongside my TRT. Bloodwork last week showed my estradiol climbed to about 55 pg/mL, and the lab range tops out near 40 for the assay they used. I dont feel the classic symptoms people talk about, no nipple tenderness, no real water retention that I can tell, maybe slightly more emotional on a bad day but that could be nothing.

For those who have run HCG longer term, anecdotally what estradiol numbers made you start paying attention? I am not looking for a protocol, just trying to understand where people generally draw the line before sides show up. I know sensitive assays and reference ranges vary a lot between labs, so the number itself matters less than the context.

I am also a bit unsure how often to retest. Right now I do a full panel every 3 months. Should I pull an estradiol specifically between full panels if I tweak the dose? My next appointment is in about 6 weeks and I plan to discuss it with my doctor, but I want to go in with a reasonable question rather than just panic at the number.

  • lifter_honest: 55 is not really a crisis number honestly. The old men who actually feel like garbage from e2 usually talk about it being well over 100, and even that is hit or miss because some guys cruise at 60 and feel great while others feel like garbage at 40. Symptoms matter way more than the lab flag in my experience? The 40 cutoff on a sensitive assay is honestly kind of arbitrary. To be fair most of the "high e2" panic on forums comes from guys chasing a number, not an actual problem. For retesting,
  • rachel266: That tracks with what ive been thinking honestly, that chasing the number seems to be where a lot of the stress comes from. I dont feel like garbage so maybe im fine. The retesting advice makes sense too, 6 weeks after any change. Right now im stable so ill just stick with the 3 month full panel and only pull a single e2 if something shifts or I tweak the dose. One thing i am curious about though, did you ever see e2 drift up slowly over time on HCG even without changing the dose?
  • tracker_jade: Anecdoally I felt fine at 55 pg/mL too, only started paying attention when mine edged past 80 and I noticed more bloating and irritability. Might be worth a quick retest in 4‑6 weeks to see if it’s creeping up further.
  • rachel266: That is reassuring to hear, thank you. Knowing you felt fine at the same level takes some of the anxiety out of it for me. The fact that yours only became noticeable past 80 gives me a bit of breathing room. I think I will take your advice and pull a retest in about 5 weeks rather than waiting until my next scheduled panel. Did yours plateau around a certain point or did it keep climbing gradually while you were on the same dose?
