Anyone tried pentosan for a cranky knee? Curious about real timelines

Got a left knee that's been bugging me for months. MRI showed some cartilage thinning and a small bone marrow lesion under the medial side. My ortho basically said "lose weight and do PT," which yeah, I'm doing both, but I'm looking for something more targeted while I rebuild.

I keep coming across pentosan polysulfate in older OA literature, specifically the work showing it can actually stimulate proteoglycan synthesis rather than just mask pain. The DMOAD angle is what caught my eye. Cartrophen-style weekly SC injections seem to be the protocol I've seen most, but oral is way more available here.

For anyone who's actually run it, what kind of timeline are we talking before the joint felt different? And did SC vs oral make a noticeable difference for you? I know the maculopathy thing with long-term high-dose oral has me a little spooked, but the cartilage data is hard to ignore.

Also, did anyone pair it with BPC-157 or TB-500, or is that overkill? Trying to figure out a sensible stack rather than throwing everything at it.

On pentosan specifically, I have not run it long enough to give you a meaningful timeline, but a colleague swears by weekly SC for his hip and says he noticed something around week 6, not before. The maculopathy risk is mostly tied to high cumulative oral doses in the European veterinary literature, which is why most people I have talked to prefer SC when they can get it.
  • freya530: Yeah the colleague timing matches what I saw in the Rastogi data, so that's reassuring. The oral bioavailability issue is what I'm hung up on, because most of what I can actually get here is oral and I don't want to run high cumulative doses just to hit a therapeutic level. Fwiw the maculopathy thing makes me nervous enough that I'd rather find a way to do SC if there's any way to source it domestically through a compounding pharmacy.   • nomad_brandon: Week 6 is kinda what i keep hearing too, for what it is worth, which makes me wonder if the people saying it did nothing just bailed at week 3. The maculopathy thing is interesting though, have you seen any actual numbers on what counts as "high cumulative" or is it just vague?
  • freya530: Honestly no, the maculopathy stuff I've seen is mostly case reports, no real dose-response curve that I trust. Most of it is from the older veterinary Cartrophen data extrapolated to humans, which isn't great. Curious where you keep hearing the week 6 thing from though, fwiw. Is that mostly forums or actual clinical reports? Because half the reason I posted was trying to figure out if 4 weeks was too early to bail.
