CagriSema week 6, food noise is gone but the constipation is real

Six weeks in on CagriSema, currently at the 1.0 step. Coming off a year on tirz first, so I figured the nausea wouldnt hit me the same. Mostly true, maybe one rough day after the jump. The actual surprise has been how quiet my brain is. Like I used to plan my next meal halfway through the current one and that loop just stopped. Ive dropped about 12 lbs so far which tracks with my calorie log.

The thing nobody warned me about is how slow everything got downstairs. I went from regular to maybe twice a week and it was getting uncomfortable. Started taking magnesium oxide at night and upped my water to like 120 oz and that has helped a lot. Also trying to get more fiber in since im eating so much less overall.

For anyone who did tirz before and switched, did your side effect profile change much on CagriSema? Curious if the amylin part adds anything beyond what the GLP-1 already does for appetite. My next bloodwork is in 4 weeks, ill post numbers if anything interesting shows up.

  • quiet_ironman: 5 step and I noticed the same quiet‑food‑noise thing – it’s like the constant snack chatter just turned off. I also hit a wall with bowel movements, went from daily to every three days and felt bloated. I switched to magnesium citrate at night, added a spoonful of psyllium husk to my morning shake and upped water to around a gallon; that’s helped a lot for me. Curious if the amylin piece is doing anything extra beyond the GLP‑1 effect on appetite, or if it’s mostly just tweaking the GI side. Le
  • rachel_ketogen: The psyllium addition is smart, I should probably add that in. I went with oxide because it was what I had on hand but citrate is probably the better move if youre doing it long term. On the amylin piece, the way I understand it is that amylin handles the meal duration signaling more directly, like it slows gastric emptying and makes you feel full longer on actual food rather than just quieting the urge to eat between meals. The GLP-1 handles the hunger and reward pathways, so theoretically the
  • patient_codes: Yeah the food noise thing is wild, it's almost unsettling at first how quiet it gets. Citrate worked way better for me than oxide too, and I do the psyllium in the morning as well. On the amylin question, honestly I can't tell what's doing what, it just feels like the appetite suppression is layered, like GLP-1 handles the volume thing and something else is smoothing out the cravings. Would love to see more on that piece specifically 🧠
  • rachel_ketogen: Ha citrate vs oxide is a good call, ill grab some next time im out and see if theres a difference. The psyllium tip is noted too, i havent tried that yet. Yeah the layered thing is exactly how id describe it too, just cant separate it cleanly. Would be cool to see someone break down the amylin contribution on its own because right now its all bundled together. Maybe a monotherapy arm somewhere down the line. Did citrate kick in faster for you or was it more about effectiveness at the same dose
  • retired_coldplunge: Fwiw the amylin angle tracks with what I have read too, the combo makes sense on paper because they really are doing different jobs. Citrate is smoother for daily use for sure, oxide can get a bit crampy if you push the dose. Has anyone tried the mag citrate liquid form or is capsules working fine for most.
  • rachel_ketogen: Yeah the citrate vs oxide thing is something I noticed when I first started mag, oxide did give me some cramping at higher doses so I backed off to 400mg and it was fine. Havent tried the liquid form though, might actually work better since its supposed to absorb faster. Did you find one worked better than the other for you?
