Pentosan for a grumbling knee, anyone run a longer oral course and actually notice the joint feel different?

Howzit. Right so my left knee has been giving me grief for the better part of a year now, the classic "fine on easy days, sore after hard days" kind of thing. MRI showed some cartilage thinning on the medial side and a bit of subchondral stuff happening. Physio helps, loaded it properly, lost a few kg, the usual.

I've been reading up on pentosan, specifically the oral side at around 100mg three times a day for a longer block. The cartilage and proteoglycan angle is what caught my eye, and the fact it might actually do something rather than just mask pain like the voltarol I keep slapping on.

For anyone who's run an oral course properly, say 8 to 12 weeks, did the joint actually feel different under load or was it subtle? And how soon before you noticed anything, weeks or months? Just trying to figure out if it's worth the patience.

  • patient_codes: Hey, knee grumble club here too ๐Ÿง  mine was the same flavor, fine on warm ups and cranky the day after heavy squats. I never ran pentosan myself but a couple of friends in coaching have done 10ish week oral blocks at roughly that dose, and the thing they kept saying was it wasn't a "wow" moment, more like around week 6 the knee stopped complaining on stairs and got quieter under load, which to me honestly sounds like the proteoglycan story playing out slowly.   • quiet_ironman: Yeah the slow burn thing tracks with what I read too, week 6ish seems to be the magic mark where people start going "oh wait, stairs are easier today" ๐Ÿ”ฌ. And that last bit nails it, if the loading program isn't dialed in alongside it you're basically just waiting on a pill to do the rehab's job.
  • rachel_ketogen: Yeah that tracks with what ive read, the proteoglycan turnover angle isnt a fast thing so the week 6 mark keeps popping up in peoples timelines. Im running a loading program alongside and dropped nightshades just to rule out confounders. Will log stair pain and knee flexion weekly to see if theres a real signal or im just chasing noise ๐Ÿ˜…
  • runner_aaron: Appreciate that, week 6 is roughly what I was working with in my head too. I've been logging my own loaded squats and step-downs weekly so I can actually compare instead of guessing. Curious on the nightshades thing, did you drop those before starting pentosan or as a baseline before? Trying to figure out if it's worth me ruling out the same confounder on my side.

My physio has me on single leg press and step downs twice a week, and the knee still nags me after the heavy sessions. One thing I'm stuck on though, did you split the doses through the day or just take it all at once?

The point about loading and cutting inflammation triggers is fair, I've been religious on the physio side but probably slack with what I eat around hard training blocks. How long ago did your mates finish their courses, and did the improvement stick after they stopped or did the knee go back to complaining once it wore off? That's the part I'm stuck on, whether it's something you ride for a few
