KFF piece on telehealth GLP-1s, medication errors piling up fast

KFF Health News has a rundown on how telehealth GLP-1 prescribing has exploded alongside the meds themselves, and so have the medication errors tied to it. Basically a deep dive on what happens when access scales faster than oversight, which is kind of the story I keep coming back to with these companies.
The thing that caught my eye was the framing around safety and compounding, not just the usual access debate. I have been tracking my own CGM and tirz numbers for months, and the providers who actually look at your data versus the ones who just push refills feel like two different worlds. The article made me wonder how much of the "errors" are patient error because nobody is coaching dosing, versus actual prescriber sloppiness.
Anyone here noticed a real difference between a clinic that monitors bloodwork and one of these cash pay telehealth setups? π

- robin589: Yeah the piece hit me too, and honestly it tracks with what i've been piecing together from lurking here. The providers that ask for bloodwork and actually look at it versus the ones that just send a script every month are night and day. My own cgm stuff only made sense once someone walked me through the numbers, before that i was just guessing. The patient error angle feels real though, because nobody explains how to handle a missed dose or when to actually titrate up, you're just told to. Tha
- tiredmira: Yeah exactly, the missed dose and titration stuff is where it falls apart for most people I think. My tirz runs were kind of a mess before I started logging everything, bloodwork plus CGM, and then comparing week to week. Without that feedback loop you're basically just guessing what the med is doing. And yeah the compounding piece is a whole separate worry. I feel like even my "research" mindset was kinda arrogant before, now I just want my numbers to look right and stay consistent π
- fasted_scientist: Honestly yeah the titration thing is spot on, ngl. I keep seeing people jump doses because the scale "stuck" for two weeks when they barely titrated properly in the first place. A mate of mine was basically on dose 2 dosing like it was dose 4 and wondering why he felt rough. Half the problem is the providers treat it like a subscription box.
- tiredmira: Right?? The subscription box comparison is exactly it. My current provider actually pulled my data before my last check-in and flagged that my fasting glucose was bouncing around, which led to holding my dose an extra week. Meanwhile I have heard horror stories of people getting auto-refilled right after their consult ends and no one even glances at the numbers. How does your mate handle it now? Did he end up switching providers or just slowing down on his own?
- weekendnate: Honestly we found him a smaller clinic that does actual intake labs, not just a questionnaire. He slowed down on his own for a couple weeks while they sorted things out. Fasting glucose creeping up was the thing that made him take it seriously.
