My first 6 weeks on oral Orforglipron – dose tweaks, nausea and step dip

I started an oral GLP‑1 a month ago after reading about Orforglipron’s oral bioavailability. My baseline weight was 185 lb, HRV around 55 ms and I was averaging 8000 steps a day. I began at 12 mg once daily for the first week to see how my gut handled it.

Day 3 I got mild nausea after dinner, but it cleared by morning. I bumped to 24 mg on day 8 and the nausea got a bit stronger – one evening I threw up after a big salad. I cut back to 18 mg for a couple of days, the vomiting stopped, and I kept the dose at 18 mg for the next two weeks.

During that period my appetite dropped by about 30 % and I lost 4 lb, but my daily steps slipped to ~6200. I’ve been tracking sleep and my REM fraction fell a bit, maybe from the lower calorie intake. I’m curious if anyone else sees a step dip with oral GLP‑1s and how you managed the nausea without quitting the dose. Any tips on timing meals or using ginger tea would be appreciated.

  • aspiring_codes: I felt kinda similar when I tried an oral GLP‑1 a few months back. The nausea hit hardest if I took it right before a big, high‑fat meal – the stomach just churned. I switched to taking it with a tiny snack (like a few crackers) about 30 min before breakfast, then waiting an hour before the main plate. Ginger tea helped a little, but what really calmed it for me was a low‑dose Pepcid 30 min earlier; it seemed to blunt the acid surge. My steps dropped a few hundred too, probably just because I f
  • amber464: That Pepcid trick sounds promising – I’ve actually tried a half‑dose famotidine before meals for acid reflux and it didn’t mess with my HRV, so I’ll give the low‑dose Pepcid a shot. I’ve been taking Orforglipron with a small piece of toast and coffee, but my breakfast is usually pretty carb‑heavy. Do you think swapping the toast for a protein‑only snack (like a boiled egg) would make a difference? Also, any specific timing you found worked best for the Pepcid relative to the pill?
  • priya_d: Ialso found the nausea worse with fatty meals. Taking it with a few crackers helped, but Pepcid made me feel jittery. My steps dipped about 300 for the first week then bounced back.
  • amber464: on the crackers – I tried that after your post and the nausea on the 24 mg day dropped from full‑blown to just a queasy feeling. I also cut back the dinner lipids a bit and it helped. My step dip was steeper (about 500) but I saw a rebound by week 4 once I settled on 18 mg. Did you end up staying at a lower dose or push back up?
