Cnbc's piece on the retatrutide obesity trial readout, and the part that caught me is how openly they flagged the nausea and diarrhea rates at the top dose. Most press releases bury that in fine print, so honestly seeing it in the lede is a nice change.

My own experience on glp-1s is that gi stuff is the whole game early on, and it tends to be dose-dependent. If the highest retatrutide arm is showing meaningful drops due to side effects, that kind of muddies the efficacy numbers people are quoting. Average weight loss means nothing if half the arm discontinued.

Curious if anyone here has tried stacking or stepping up slower than the trial protocol to manage this. Also, did the article say anything about lean mass preservation, because that is what i would actually want to see next.

  • daily_sleep: Yeah the gi stuff is honestly the whole story with this class of drug... When i tried a glp-1 years back the nausea in the first two weeks was rough, basically had to time my meals really small and early. Stepping up slower made a big difference for me, but that is just my own experience... On the lean mass point, agree, that is the bit i keep waiting for.   • calmcasey: Yeah the lean mass thing is exactly where my head goes too. Scale weight is a useless endpoint if youre losing muscle along with fat, especially for people who are already training hard. The trial data should at minimum show dexa or dwa breakdowns by arm, and i havent seen that yet. The small early meals trick worked for me too, honestly that and slowing the titration were the two things that got me through the first month without wanting to quit.   • nora793: Ok but hear me out, if you have to eat like a medieval peasant to stay on a glp-1 is that even a win? Like congrats you lost weight by being too nauseated to function. Fwiw the lean mass thing is what bugs me too, every study just throws the scale number around like it means something. Body comp or it didn't happen, right?
  • calmcasey: Yeah thats kind of my point, the eating like a peasant thing stops being a joke and starts being the actual restriction most people end up at. If youre forcing down plain rice and chicken because anything else makes you sick, the weight loss isnt really a triumph of the drug, its a triumph of white knuckling it. And honestly the lean mass angle is the part im watching for the next data drop, because if youre dropping 25% of your weight and a quarter of that is muscle, you havent fixed anything,
