TB-500 alone vs stacked with BPC-157 for a stubborn patellar tendon, what worked for you?

Hey, so I've been nursing a cranky patellar tendon on my left knee for the better part of 4 months now. It's that classic "annoying on stairs and deep squats, fine at low loads" kind of thing. I've done the usual boring rehab, isometrics, heavy slow eccentrics, deloads, the whole boring list. Progress is there but slow, tbh.

I'm now weighing whether to add TB-500 on its own or just run it with BPC-157 from the start. I've used BPC before for an adductor strain and felt it moved the needle, but the tendon feels like a different beast, less blood flow, more stubborn.

For those who've actually run TB-500, did you notice anything specific on tendon tissue or was it more of a muscle thing for you? And if you stacked it with BPC, did you run both daily or spread them out? How long before you felt something real on the tendon, not just "I think it's better" placebo vibes? Keen to hear what actually worked, not theory.

  • sleep_nerdy: Ngl tendons are the worst, they take forever and it's hard to tell if anything is actually helping or if you're just waiting out the clock. I've used both on a nagging achilles thing a while back, ran TB-500 twice a week and BPC daily. Couldn't honestly tell you which one did the work or if it was just time, the thing cleared up over like 8 weeks. Felt "warmer" in the area if that makes sense, but that could easily be placebo.   • greg208: Not gonna lie the imaging point is spot on, 4 months of slow progress on a patellar tendon is worth getting checked before you chuck research compounds at it. Could be tendinopathy but could also be a partial tear or something else entirely and youd want to know that first. Did the achilles feel noticeably different week to week or was it more like you woke up one day and realised it was fine?
  • tiredmira: Yeah 8 weeks is basically the textbook "wait it out" window for tendons which is why it's so hard to separate 😅 The warmer feeling, I kinda get that too when I ran them stacked for a rotator cuff issue. Could've been bloodflow, could've been me wanting it to work tbh. Imaging is a good shout, echo or MRI would tell you if there's actual degeneration vs just tendinopathy. Changes the whole approach.
  • runner_aaron: Tbh the 8 week thing is what makes me hesitant to just keep grinding isometrics and hoping. Rot cuff is interesting though, did you stack both daily or split them? And how long before you actually felt the difference on the shoulder, not just "warmer" but proper load tolerance?

That's partly why the patellar one is bugging me, no clear week to week shift yet, just background noise. Point taken on imaging though, I've been putting it off because I assumed it's just tendinopathy. Howzit work where you are, do you just go to a sports doc and ask for an MRI or do you push for ultrasound first?
  • runner_aaron: Yeah fair point on the imaging, tbh I've been putting that off because part of me doesn't want to know. Achilles is interesting though because the blood flow story is similar. How long were you on each before you noticed the warmer feeling, and did you keep BPC going after you stopped TB or drop both together?
