TB4-Frag plus oral BPC-157 for shoulder tendon strain – dosing around physio?

I’ve been dealing with a nagging shoulder tendon strain that started after a heavy lift during training, and as someone who’s used to pushing through pain I wanted to try something that might actually help the tissue heal rather than just mask it. I ran a short course of TB4-Frag at 400 mcg every other day subcut, and alongside that I started oral BPC-157 (arginate salt) at 500 mcg each morning on an empty stomach. I kept my physio sessions twice a week, focusing on eccentric loading and mobility work, and I tried to time the peptide shots a few hours before each session so the peak would line up with the workout.
For me the soreness seemed to dip a bit after the first week and I could move through the range with less sharp pain, though it could just be placebo or the extra rest I was giving it. Has anyone else stacked TB4-Frag with oral BPC-157 for tendon issues, and how did you schedule the doses around your rehab? Any tips on adjusting the frequency if you notice fatigue or mild headache? Would love to hear what’s worked or not worked for you all.

- elle_z: I tried a similar combo for a nagging elbow tendon strain last year – TB4-Frag 300 mcg subcut every other day and oral BPC-157 500 mcg in the morning. I timed the injection about 4‑5 hours before my physio sessions so the peak lined up with the eccentric work. After about ten days the dull ache felt a bit less sharp and I could push through the rehab with less wincing, though I can’t rule out placebo or just giving the joint more rest. I did get a mild headache once or twice; dropping the TB4-F
- fasted_scientist: Ngl, I ran a similar TB4‑Frag 300 mcg EOD with oral BPC‑157 500 mcg for a knee tendon niggle. Felt a bit less soreness after a week, but I also cut back on heavy loading and hydrated more, so hard to say what moved the needle. Any bloodwork or markers you checked?
- fasted_scientist: Ngl, I’ve seen similar mild headaches with TB4‑Frag when I pushed the dose a bit higher – upping water and splitting the dose helped for me too. Curious if you checked blood pressure or just put it down to dehydration? Just my own experience, of course.
- casey613: Yeah I just chalked it up to not drinking enough, didn’t check BP.
- casey613: I didn’t run any bloodwork or markers, just going by feel and physio notes. Did you notice any changes in sleep or appetite while on that stack? Curious if those tweaks mattered for you.
- casey613: That is steady, you did the 4 to 5 hour window too. I have not had any headaches with my 400 mcg dose, but I do drink a lot of water anyway because of the humidity here. Since you dropped your frequency to twice a week to fix the headaches, did you feel like the recovery speed slowed down for your elbow, or did it still feel the same?
