Oral Pentosan Polysulfate for knee cartilage – worth a try?

Hey folks, I’ve been dealing with a nagging ache in my left knee after my last 50k ultra. It’s not sharp pain, more like a deep soreness that lingers for days and flares up on downhill sections. I’ve been running BPC-157 oral at 500 mcg twice a day for the past three weeks and it’s helped with gut comfort but the knee feels about the same.
I came across Pentosan Polysulfate (the oral version) while looking into cartilage support and saw it’s used for osteoarthritis and has that proteoglycan synthesis thing. Has anyone here tried it for joint soreness from endurance training? I’m thinking about giving it a shot at the typical 100 mg three times daily but wanted to hear if anyone noticed any difference in recovery time or just general joint feel.
Also, any GI upset to watch out for when stacking it with BPC-157? Ngl I’m cautious about the bleeding risk since I’m already on a low dose aspirin for occasional headaches. Would love to hear your experiences or any timing tips around physio sessions. Thanks!

- weekendnate: I’ve looked into oral pentosan polysulfate after a similar nagging knee soreness from a long trail run. I tried a low dose for a couple of weeks while keeping my usual BPC‑157 routine and didn’t notice a clear change in joint comfort or recovery time, just my own experience, could be placebo or that my issue was more tendon‑related than cartilage. I did keep an eye on any stomach upset or bruising; nothing stood out, but I’m also on aspirin occasionally so I spaced the doses apart and avoided ta
- quiet_ironman: I tried oral pentosan polysulfate at 300 mg daily for three weeks alongside my BPC‑157 and didn’t feel any change in knee soreness either, though my stomach stayed fine. Did you notice any difference in swelling or just comfort?
- elle_z: Hey weekendnate, I haven’t tried PPS yet but was thinking 300mg daily with BPC‑157. Did you notice any difference in swelling or just soreness? Tracking symptoms definitely helps separate placebo from real effect.
- grinder265: Ngl the swelling question is a good one. Honestly I havent tracked swelling super closely, its more that deep achy feeling that hangs around after long descents. Maybe a little puffiness on real bad days but nothing dramatic. I might start logging morning vs evening knee circumference just to get a real read. 300mg daily is interesting tho, most ppl I see run the higher split dose. Any logic to taking it all at once vs spreading it out?
- grinder265: Yeah the lack of noticeable change is exactly what scares me away from throwing 300+ mg at this. For me the knee isnt visibly swollen, its more of a stiffness behind the kneecap that worsens on downhills. Did you stack anything else with the pentosan or just run it solo with the BPC? And out of curiosity did you stop after 3 weeks or push it longer before calling it?
- grinder265: Appreciate the honest take. The tendon vs cartilage angle is something i hadn't really considered, my achilles stuff has always been more tendon-feeling but this knee thing is deeper and achier, kinda sits behind the kneecap on downhills. I'll probably still try a run of it just to see what happens for me but i'll keep expectations in check. Good call on spacing from the aspirin, ngl i was just gonna take everything at once. How long did you run it before deciding it wasnt doing much?
- nerdy_macros: Honestly I dont have strong evidence either way on once vs split for PPS. The half life is short so pharmacokinetically splitting makes sense, but I havent seen head to head data comparing joint outcomes. Might be worth starting split and seeing if you notice anything different. Tbh if the deep ache is the main thing, swelling tracking is a good call either way 👀
