How I timed oral BPC‑157 with physio for a stubborn rotator cuff strain

I’ve been dealing with a nagging rotator cuff strain for about six weeks now – the kind where the pain spikes after a long coding session at the desk and then eases a bit after a light stretch. A few months back I tried oral BPC‑157 (stable arginate salt) for a gut issue and liked the convenience, so I thought I’d give it a shot for this shoulder problem. I started at 500 mcg once daily on an empty stomach, then moved to 500 mcg twice a day (morning and before bedtime) after the first week because the pain didn’t seem to improve much.

I also began seeing a physical therapist twice a week, focusing on eccentric rotator cuff work, and I made sure to take the morning dose at least an hour before the first session and the evening dose right after the final session. After about three weeks I noticed the ache during overhead lifts was noticeably less, and my range of motion improved by a few degrees. I did get a mild nausea the first two days, but it settled down.

Has anyone else tried timing oral BPC‑157 around rehab sessions? Did you stick with once‑daily dosing or go twice? I’m curious if the timing really makes a difference or if I’m just riding a placebo wave.

  • sanjay750: ngl it could be placebo but the timing makes sense to me. i used the stable salt for a bad ankle sprain a while back and did the same thing. i went with twice daily doses because it seemed to keep the inflammation down better than just once. i didnt really feel a huge difference in timing around my stretches, but the overall recovery felt faster than when i just did physio alone. did you notice if the nausea got worse when you bumped the dose to twice a day or was it just the first few days reg
  • paul_rows: I didn’t get any extra nausea when I upped it to twice a day – the mild queasy feeling was just the first 24‑48 hours after the initial 500 µg and then it cleared. After that the two‑dose schedule felt fine, no stomach upset even on the evenings when I took it right after PT. As for joint targeting, I can’t prove it but the fact that the pain drop line‑up with the PT sessions (morning dose before the workout, night dose after) makes me think there’s at least a systemic anti‑inflammatory effect t
  • biohacker_priya: I didn’t notice more nausea when I went to twice‑daily BPC‑157; the mild queasiness only lasted the first couple days either way. Hard to say if it’s hitting the joint or just systemic, but the pain did feel a bit steadier on the split dose.
  • paul_rows: Thanks for the heads‑up on the nausea, yeah, I felt the same brief queasy spell the first two days regardless of dose. I did notice the split dose seemed to keep the ache more consistent between physio sessions, like you said. Do you ever time the evening dose right after the day’s last PT work, or do you leave a bigger gap? I’ve been doing it right after, but wonder if a longer window changes anything.
