Staying on top of hematocrit after Sustanon 250 injections

I started Sustanon 250 at about 250 mg every three weeks a couple of months back because my GP thought it was a safer TRT option than the usual testosterone cypionate. After the first couple of doses I noticed a faint pallor in my fingers and a bit of fatigue – not ideal for a retired firefighter who still does a bit of manual work. A quick check at the local clinic showed my hematocrit at 46 %, a touch above the typical upper limit of 44‑45 % for a man my age.

I didn’t expect that rise, and I’m a bit concerned about the risk of blood clots or higher blood pressure. I’ve read that Sustanon intuitively can push red cell mass up because of the testosterone spikes from the short‑ester mix. I’ve started taking a week‑long low‑dose iron supplement to see if that helps keep the numbers steady, but I’m not sure if that’s the right approach.

Has anyone else seen a similar hematocrit jump, and how often did you check the levels? Any tips on how to keep it in a safe range while still getting the benefits?

  • owen196: I’ve been watching my hematocrit after starting Sustanon for a year now. I pull my CBC every 4‑to‑6 weeks right after the injection. When I hit 46‑48 % I stop the iron‑value supplement for a week and add a few extra grams of water each day. Hydration pulls the plasma volume up and keeps the red cell fraction down. I’ve never needed a formal EPO test; the trend usually just levels out. If you’re feeling light‑headed or paler, a quick dip on a phone analyzer is a good mid‑cycle check. Keep the do
  • greg208: Thanks owen, that makes sense – I was worried the iron was doing more damage than help. I’ll try raising my water intake by a litre a day and stop the iron when my CBC tops 47 %. Do you mind how many extra grams you add daily? Also, have you noticed any change in how you feel after the hydration tweak?
  • codes_nomad: I’ve also noticed that drinking an extra liter or two on injection day seems to blunt the hematocrit creep for me, though I can’t say it’s not just placebo. Have you checked ferritin when you pause the iron supplement? Curious if that’s part of the balance.
  • greg208: Thanks for the tip, codes_nomad. I’ve only just started the low‑dose iron, so I haven’t checked ferritin yet – will get a test next week. The extra water seems to help; I’ve been shovelling a litre or two on injection days. Do you notice any haematocrit drop after that veux?
